What is ACCESS?
The ACCESS Program is a CMS Innovation Center model that expands access to technology-supported care for people with chronic conditions. Participating Medicare-enrolled organizations provide ongoing support, such as education, coaching, medication management, remote monitoring, and care coordination, to help people improve their health alongside their existing care team.
Is there a cost to participate?
ACCESS services are available at low ($0-$7/month) or no additional cost for eligible people with Original Medicare.
Will this replace my doctor?
No. The ACCESS Program works alongside your existing healthcare providers to offer additional support, education, and guidance between visits.
Who is eligible?
ACCESS is currently available to eligible people with Original Medicare who have qualifying chronic conditions, including diabetes, high blood pressure, chronic kidney disease, chronic musculoskeletal pain, depression, and other conditions included in the program's clinical tracks. Eligibility is determined during the enrollment process.
How is my information protected?
Your information is securely handled in accordance with healthcare privacy standards and is only used to determine eligibility and support your participation in the ACCESS Program.
