A plan that fits you. We build a plan around your goals and adjust it as things change. If you already have one from your doctor or therapist, we work with that.
Education. Learn what helps with your condition and why, in plain language.
Coordination with your care team. We keep your doctor in the loop, with your permission. This works alongside the care you already get, it doesn't replace it.
Ongoing support. We work with you month after month, so the changes you make actually last.

What is ACCESS?
The ACCESS Model is a CMS Innovation Center model that expands access to technology-supported care for people with chronic conditions. Participating Medicare-enrolled organizations provide ongoing support, such as education, coaching, and care coordination, to help people improve their health alongside their existing care team.
Is there a cost to participate?
There is no cost to you for our services. We have waived the Medicare cost-sharing, so you pay nothing out of pocket for the support we provide. Your regular Medicare coverage and what you pay for other care do not change.
Will this replace my doctor?
No. The ACCESS Model works alongside your existing healthcare providers to offer additional support, education, and guidance between visits.
Who is eligible?
You may be eligible if you have Original Medicare, live in Florida, and have ongoing musculoskeletal pain (such as back, neck, knee, hip, or shoulder pain) or depression or anxiety. We confirm eligibility during sign-up.
How is my information protected?
Your information is securely handled in accordance with healthcare privacy standards and is only used to determine eligibility and support your participation in the ACCESS Model.
What if I need help right away?
This is not an emergency service. If you need help right away, call 911, or call or text 988 for mental health support.
Do I need a referral from my doctor?
No. You can sign up yourself.
